Code 6. Level I Screen


Includes all activities related to performing a Level I Screen.  
This includes the following activities:

  • Time spent on the telephone, in–person, or via a website obtaining information to fill out a Level 1 Screen.
  •  Time spent contacting additional individuals, such as physicians or other family members, to complete or verify information included on a Level 1 Screen.
  • Time spent traveling to and from a Level 1 Screen that is conducted in person.
  • Time spent conducting administrative activities necessary to complete a Level 1 such as:
    •  Identifying correct contact information.
    • Entering data into an electronic system.
    • Answering questions about the purpose and nature of the Level I Screen.
    • Providing results of the Level 1 Screen and making appropriate referrals.
  • Setting up translation or signing services.

This code should be used for all activities related to the Level I Screen, including:

  • Paperwork
  • Clerical activities
  • Time spent reviewing voice mails or emails
  • Time spent consulting with a supervisor or supervising 
  • Travel including time spent arranging transportation
